Program Overview
The Home Study and Post-Release Services (HS/PRS) program supports unaccompanied children released from federal custody to sponsors across the U.S. The goal is to ensure the safety and well-being of children through pre-release home assessments and post-release case management. This role is funded under a 36-month project running through September 30, 2026.
Position Summary
The Crisis Case Manager (Level 3) provides trauma-informed, intensive case management to children and sponsors facing serious challenges or instability. Responsibilities include conducting in-person home studies and ongoing post-release case management, with a focus on safety, mental health, legal access, and family stability.
Responsibilities

Conduct in-home assessments and weekly follow-ups for first 45–60 days

Complete home study reports within 10 days of referral

Develop individualized service plans and assess client safety and needs

Respond to crises involving abuse, trafficking, or safety concerns

Provide referrals and connections to:

Legal services and guardianship assistance
-
Education and ESL programs
-
Health and behavioral care
-
Substance use or gang prevention programs
-
Family counseling and youth development services

Ensure all case documentation is accurate and submitted on time

Maintain confidentiality and adhere to mandatory reporting guidelines

Travel up to 80% (including weekends/evenings) within 200-mile range
Qualifications
-
Education: Master’s in Social Work, Psychology, or related field (with direct clinical experience)

Experience:
-
Minimum 2 years of case management, with at least 1 year in child welfare or post-release services
-
Experience working with immigrant youth or unaccompanied minors highly preferred

Language: Fluency in English and Spanish (written and spoken)

Skills:

Strong organizational and crisis intervention skills

Proficiency in Microsoft Office Suite, Zoom, Teams

Ability to work independently and in high-stakes environments

Familiarity with local social service systems

Conditions & Requirements

Must pass FBI background check, fingerprinting, and Child Abuse/Neglect clearance (facilitated by IRC)

Must have valid driver’s license and reliable transportation

Private and secure workspace required if remote
